Everything You Need for Recording Work-Related Injuries

From recording the accident—including witnesses and circumstances—to the documentation for the report and corrective actions.

Injury Report

Date, time, location, description of the incident, and type of injury.

Witnesses and Circumstances

Who witnessed the accident and what led to it.

On-site Measures

What was done immediately and what corrective measures will follow.

Supporting Documentation for the Report

Information for the accident report, the insurance company, and the labor inspectorate.

Documents and History

Photos, medical reports, and every change to the record.

Reports and Statistics

Injuries by workplace, time period, and type of injury.

Problems vs. Solutions

The injury log begins with a complete record

An employer must maintain an accident log and, in the case of more serious accidents, prepare a report and file it. Paper and spreadsheets can handle this, but it’s hard to track who entered what and when.

Problems

Paper Book

The record is hard to find, difficult to read, and sometimes even gets lost.

Incomplete data

There are no witnesses, no details of the circumstances, and no measures have been taken.

Deadlines in Mind

One person is in charge of tracking the deadlines for accident reports and notifications.

The measure will be lost

No one is checking to see if the correction was actually made.

No overview

You don’t know where and why injuries keep happening.

How SmartFP solves it

One Accident Log

All accidents and incidents in one place, searchable in a second.

Structured Record

The form serves as a record, so neither the witnesses nor the measures are missing.

Supporting Documentation for the Report

You’ll have all the information you need for an accident report right at your fingertips when you need it.

Monitoring of Measures

Each corrective action has an assigned person, a deadline, and a status.

Reports

Injuries by workplace, period, cause, and type of injury.

How the Injury Log Works in Practice

Date, time, location, description of the incident, and type of injury. Even a shift supervisor can handle this on-site.

The form is complete; nothing is missing.

Risks by Workplace

What is an injury log?

A record of all workplace accidents, which the employer is required to maintain under the Labor Code.

When is an accident report filed?

If an employee is unable to work for more than 3 calendar days, or in the event of death. The module generates the report based on the data in the ledger.

Who can report an injury?

The person in charge, the department head, or the employee themselves, depending on their assigned permissions.

Can I see who edited the entry?

Yes. Each edit includes the date, the user, and a description of what was changed.

Can witnesses and the circumstances be documented?

Yes. When reporting the accident, include the witnesses, the circumstances that led to it, and the measures taken at the scene.

Can I submit documents related to the accident?

Yes. Please attach photos from the scene, medical reports, or incident reports directly to the record.

How does the module help with the investigation?

You have the sequence of events, witnesses, actions taken, and documents all in one place. You don’t have to search for anything.

Do we have to rewrite the old book?

No. You can import older records, but you only need to enter new ones directly into the module.
